Its physician-lead multidisciplinary care teams include physicians, nurse practitioners and other clinical providers who provide a broad range of services via telehealth. The Nurse Practitioner collaborates with the medical team in the development of the comprehensive plan of care, assessment of patients, and participation in the identification of eligibility for Remote Patient Monitoring.
Essential Duties and Responsibilities:
This is a fully remote, part-time position as an independent contractor.
Candidates must be willing and able to provide a minimum of 10 hours of availability per week, up to 20 hours weekly
Provides consultation, problem solving, guidance and education to assure optimal remote patient monitoring care for patients and their supporting families.
Active participation in meeting customer satisfaction goals.
Always putting the patient first and providing extraordinary empathy
Engages in immediate, frequent collaboration with the Medical Director or Chief Medical Officer to share patient progress and plans of care, as well as active problem solving.
Document, Charge Entry, and Participate in Chart Audits to ensure accuracy and billing and compliance.
Collaborates with the Attending Physician as needed to maintain an effective plan of care that produces excellent outcomes for the patient and family.
Promote a positive culture among and across the Aurkei Clinical team.
Promote teamwork and shared responsibility for attaining the agency mission and vision.
Actively engage in QAPI initiatives and compliance activities.
Take part in professional development to maintain qualifications for role; completes CBTs in a timely manner.
Conducts virtual telehealth assessments and plans as necessary. The Nurse Practitioner evaluates clinical evidence for eligibility and provides this information to the Medical Director and clinical care team.
Assists the clinical team in the development of the comprehensive assessment of patients and families.
Communicates and collaborates with the Medical Director and obtains their input in the care of patients/families.
Occasionally provides medical services for patients as directed by the care team when the primary provider is unavailable or wishes for Nurse Practitioner support.
Provides input in the definition and measurement of meaningful outcomes that assess effectiveness of interventions and medications.
Readily offers suggestions that promote continuous improvement of outcomes and the safe provision of care.
Assures that all elements of performance meet the Medicare Remote Patient Monitoring or Chronic Care Management Guidelines.
Maintain productivity standards.
Assures appropriate medical care to achieve clinical and financial outcomes.
Is an ambassador for the agency in daily encounters with referral sources.
Promote positive relationships with customers.
Adheres to all company policies and procedures.
Adherence to and compliance with information systems security is everyone’s responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered. Report information systems security problems.

Minimum Qualifications:
Education/Licensing/Certification:
Master’s Degree in Nursing.
Current state Registered Nurse licensure.
Nurse Practitioner certified (or ability to sit for certification exam within three (3) months) through ANA or equivalent.
Current state Advanced Practice Nurse Prescriber certified or eligible for certification.
Must be CPR certified.
Experience:
Five years nursing experience
Previous experience working as an Advance Practice Nurse.
Experience in Remote Patient Monitoring or Chronic Care Management is preferred.
Knowledge, Skills, and additional requirements:
Comprehensive knowledge and skills in current nursing practice, and the ability to work with a high level of autonomy and independence.

Flat rate of compensation for each patient consultation
