We are searching for a Case Manager to demonstrate the ability to handle confidential information with discretion and ability to deal with the people in a professional and courteous manner. Ability to meet deadlines, manage multiple priorities and enhance the spirit of teamwork through effective role modeling. Excellent interpersonal, communication and organization skills. ABOUT THE AREA:

Penn Yan is an incorporated village in Yates County, New York. It is the county seat of Yates County and lies at the north end of the east branch of Keuka Lake, and just west of Seneca Lake. This area is known as the Finger Lakes, also known as wine country.
You will share the roads with Amish and Mennonite Buggies through the beautiful hills. This is a wonderful place to be in the summer. Not far you can find all kinds of shopping, outdoor sports, waterfalls, hiking, parks and more.

Requirements: REQUIREMENTS:

Graduate of an accredited school with Bachelors Degree in Social Work or Psychology

At least one years experience working with individuals with a psychiatric illness or related experience in case management services.

Valid NYS driver's license clear of violations in the past five years

Must take BLS and CPI training within 1 year of hire.

NYS Case Management Certification within 2 years.
