Job Description:

This RN Case Manager will work specifically with our neuro-oncology pediatric patient population in Rady Children's - Peckham Center for Cancer & Blood Disorders -outpatient department.
 

JOB SUMMARY:

Peckham Center Case Manager coordinates provision of care across the continuum for pediatric patients with a hematology and/or oncology diagnosis. The Case Manager advocates for the patient and family through triage, education, and navigation of insurance authorization requirements, as appropriate, to the nursing role. The Case Manager identifies, accesses and allocates resources to meet the goals of care, as identified by the multidisciplinary team, in partnership with the patient and family.
 

MINIMUM QUALIFICATIONS:

Bachelor's Degree

3 years of experience

California RN License

CPR/BLS Certification (AHA)

Must qualify for CCS Paneling

Must have good problem-solving skills

Excellent oral and written communication skills

Able to manage large call volumes and triage patient care needs appropriately

Experience working with children and adolescents with hematology and/or oncology diagnoses
 

PREFERRED QUALIFICATIONS:

Master's Degree

5 years of experience

3 - 5 years Peds HemOnc experience

Case management and NeuroOncology experience

Bilingual - English/Spanish

Shift: Day

FTE: 1.0FTE, Full-time, Benefit eligible

Location: Main Campus, Acute Care Pavilion (San Diego, CA)
