PHYSICAL THERAPIST

Contract Length: 13 weeks
 
Guaranteed Hours per Week: 40
 
Primary Location: Iron River, MI
 
Shift Requirements: M-F 8 am-4.30pm
 
Float Requirements:

• Iron River primary work area; work in adjacent teams as needed (Ironwood).
• If they needed to help the Ironwood team, they would be helping in the areas that just border the Iron River team (would not travel far outside normal radius) (mileage reimbursement)

• AHA BLS, Licensure required for the State of Michigan.
• WI License is desired, but not required
   

• Master's Degree in Physical Therapy.
   

• Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis.
• Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.
   

• Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication.
   

• Home Health Average Daily Census in Iron River 57.8 and we currently have 1 PT’s and 2 PTA to cover the area, but this contract position is to cover a maternity leave for the PT.
   

• Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice.  
• Population served includes adult and geriatric patients.  
• Documents to meet regulatory and reimbursement requirements.  
• Meets productivity expectations of an average of 5.2 pts per day.
• Hard worker, Resilient and Manages Change well, Social candidates.
