Korean Bilingual Social Worker

Basic Duties / Job Summaries:

• Interview and screen all referrals to determine the general appropriateness of the prospective participant for the full assessment process and Adult Day Health Care (ADHC) participation.
• Provide referral for persons not appropriate for ADHC.
• Provide a signed and dated documentation for all service performed the day the service is provided and include signed and dated quarterly progress reports in each participant record.
• Provide for initial evaluation and periodic reevaluation and revision of plan of care.
• Provide case management and make referral to available community resources.
• Provide 1:1 and/or group counseling through problem-centered and/or person-centered care planning.
• Serve as liaison with the participant’s family and home.
• Serve as liaison with other community agencies that may be providing services to a participant and work with agencies to coordinate all services based on participants needs.
• Provide discharge planning for all participants.

Required Qualifications:

• Preferred ADHC/CBAS experience
• Bilingual in Korean and English
• Master Degree in Social Work or Psychology, Gerontology or Counseling or related-field OR
• Bachelor Degree in Social Work or Psychology, Gerontology or Counseling or related-field
• Basic knowledge of MS office including Word & Excel
