Job Description:
· Under the guidance of a RD
o Perform Admission and High Risk Assessment, as well as QA Food Service Audits.
o Perform MDS, care plans, schedule case, etc. at facilities
o Meeting with staff for recommendations or processing information.
o Survey preparations for annual state survey
o Customer Service and relationship building
o Update facilities notebook with tracking forms
