Responsibilities

• Providing hygiene care as directed by a nursing supervisor, including but not limited to bathing, grooming, oral care, dressing, undressing, toileting activities.
• Providing assistance with daily tasks, including but not limited to feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shopping
• Assisting client with active and passive mobility work defined by the plan of care, and documenting observed changes to the Director of Nursing
• Assist client in active and passive range of motion defined by the plan of care, document observed changes, report any changes to the Director of Nursing
• Develop and promote a safe environment for quality client care through adherence to established policies, procedures, and standards
• Ensure client’s rights are adhered to

Requirements

• Must provide a copy of current negative TB skin test or chest x-ray; or able to get one
• Must be 18 years old
• Minimum one (1) year of professional documented experience
• Current CPR Certification
• Licensed driver with insurance and reliable automobile with proof of insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds.
• Adhere to HIPAA and maintain client confidentiality

Example of hours:

Neenah - Mon, Wed, Fri 9a-10a every other Sun 9a-3p.

Neenah - Tues, Wed, Thurs, Fri, Sat, Sun 8a-10a

Appleton - Wed 10a-12p; Thurs 10a-1p; Fri 7p-9p, every other Sat and Sun 8a-10a and 7p-9p
