Case Manager - Foster Care

Seeking an experienced full-time Foster Care Case Manager in the Philadelphia, PA area.

Responsibilities include:

• Provide support: Offer emotional support and counseling to children and families 
• Develop care plans: Create individualized care plans for foster children 
• Coordinate services: Link families to resources and services that improve quality of life 
• Conduct home visits: Monitor the child's progress and well-being including regular safety check visits
• Complete documentation: Complete mandated reports and documentation & maintain visit notes 
• Provide guidance: Provide guidance and direction to foster and adoptive parents 
• Oversee homes: Oversee and ensure compliance of assigned foster and adoptive homes 
• Help develop life skills: Help foster children develop life skills that they need in life outside of foster care 

• Bachelor's degree in Social Work or related field
• At least 2 years of experience as a Social Worker
• Preferred locations: Delaware, Chester, Montgomery & Philadelphia Counties
