Care Manager positions are located throughout Queens, Brooklyn and Long Island, NY.
Care Manager Requirements (Full-time) Monday – Friday (9A-5P):
• Must have 2 years Case management experience.
• Must have BA/BS with 2-years of relevant experience working directly with people with physical and/or intellectual disabilities
• OR a license as a Registered Nurse with two years of relevant experience,
• OR a Master's degree with one year of relevant experience.
