Job Summary:
Shriners Hospitals for Children- Portland is seeking an on-call Social Worker to join our Care Management team! This position requires understanding of common ethical and legal issues in social work practice in health care settings including biopsychosocial–spiritual assessments, use of the strengths perspective, client and family engagement in all aspects of social work intervention, and case management/ care management/ care coordination/ health care navigation.
Responsibilities Include:
- Discharge and transition planning
- Client concordance with and adherence to the plan of care
- Advanced care planning, palliative care, including pain and symptom management, hospice and end-of-life care
- Identification of child abuse, trauma, neglect, and exploitation
- Crisis intervention
- Facilitation of benefits and resource acquisition to assist clients and families, including an understanding of related policies, eligibility requirements, and financial and legal issues
- Advocacy with other members of the interdisciplinary team and within the health care institution to promote clients’ and families’ decision making and quality of life
- Client, family, interdisciplinary, and community education
- Addresses family systems issues, including the impact of health care concerns, illness, and disease on family relationships
Knowledge, Skills & Abilities:
- Master’s Degree in Social Work is required
- 3 years’ experience as a Social Worker is required, 5-7 years preferred, experience in a Pediatric setting is preferred
- Current Social Work License in the State of Oregon
- Case Management Certification (CCM, ACM, or ANCC) is required or obtained within 6 months of hire or eligibility
- Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers is required
- Experience in a healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/ family adherence is preferred
- Demonstrates strong communication skills - both oral and written
- Excellent customer service and service recovery skills is required
- Demonstrates desire for and potential for growth and professional development; The ability to manage conflicting priorities is required
- Demonstrates empathy, caring and compassion for children with complex medical conditions and their support systems
- Proficient in Microsoft Office (Word, Excel, PowerPoint and Outlook)
- Demonstrates willingness to support efforts to maximize the efficiency and effectiveness of the electronic medical record
- Bilingual in English and one additional language is preferred
