Shifts are one to one inside the patients home. Nurse should have experience with G-Tubes, Trach's or Ventilators.

JOB DESCRIPTION SUMMARY: THE LICENSED PRACTICAL/VOCATIONAL NURSE IS RESPONSIBLE FOR PROVIDING DIRECT PATIENT CARE UNDER THE SUPERVISION OF A REGISTERED NURSE. RESPONSIBILITIES INCLUDE FOLLOWING THE PLAN OF CARE, PROVIDING TREATMENTS, AND WORKING COLLABORATIVELY WITH THE MEMBERS OF THE TEAM TO HELP MEET POSITIVE PATIENT CARE OUTCOMES.
 

• PROVIDE DIRECT PATIENT CARE AS DEIFNED IN THE CALIFORNIA NURSE PRACTICE ACT. IMPLEMENTS PLAN OF CARE INITIATED BY THE REGISTERED NURSE AND PRIMARY CARE PHYSICIAN.
• PROVIDES ACCURATE AND TIMELY DOCUMENTATION CONSISTENT WITH THE PLAN OF CARE.
• CHARTING/DOCUMENTATION MUST BE COMPUTER-LITERATE.
• PARTICIPATES IN COORDINATION OF HOME HEALTH SERVICES, APPROPRIATELY REPORTING THE IDENTIFIED NEEDS FOR OTHER DISCIPLINES (HHA, OT, PT, MSW, ST, DIETITIAN) TO THE REGISTERED NURSE AND/OR CLINICAL SUPERVISOR/NURSING SUPERVISOR.

• POSITION QUALIFICATIONS: GRADUATE FROM OF AN ACCREDITED VOCATIONAL NURSING PROGRAM. HAS ( 1 ) YEAR OF NURSING EXPERIENCE. CURRENTLY, LICENSED AS A VOCATIONAL NURSE IN THE STATE OF CALIFORNIA. COMPLIES WITH ACCEPTED PROFESSIONAL STANDARDS AND PRACTICE.
• DEMONSTRATES GOOD VERBAL AND WRITTEN COMMMUNICATION, AND ORGANIZATIONAL SKILLS.
• POSSESS AND MAINTAINS A CURRENT CPR CERTIFICATIONS, TB/CHEST X-RAY AND PHYSICAL EXAM. MUST BE A LICENSED DRIVER WITH AN AUTOMOBILE THAT IS INSURED IN ACCORDANCE WITH STATE/OR ORGANIZATION REQUIREMENTS AND IS IN GOOD WORKING ORDER.
• ABLE TO WALK, BEND, STOOP, AND LIFT OBJECTS WEIGHING UP TO 25-35 LBS. MUST BE COMPUTER-LITERATE, HAVE A SMARTPHONE, TABLET, LAPTOP, ETC IN ORDER TO DO EMR DAILY CHARTING.

We are also contracted with CHOC Health Alliance, LA Care, and various other managed care health plans. We also accept Respite Cases with specialization in Sub-Acute Pediatrics that allow patients to apply for the Medi-Cal EPSDT program. We also serve all other age groups such as the NF/AH (HCBS) Waiver Program.

Exceptional Home Care is now using Electronic Medical Record Charting online in order to provide a more proficient way in documenting/charting while at the patients home. All nurses will be trained on the EMR Devero/Netsmart system before beginning their shift care assignment.

We are currently looking to hire Full-Time and Part-Time LVN's with at least one year of LVN experience for our patients within the Los Angeles and Orange County areas. We are offering competitive pay rates, benefits, and flexible schedules. Our patients are in need of LVN's to provide 6-12 hour shifts of one on one total care inside the patients home. Our patients either have a G-Tube, Tracheostomy, Ventilators or Oxygen. LVNs will need to administer medications, administer feedings, provide ROM, provide Peri-care, Assist with ADL's, transfers/repositioning of patient. 

We are also looking to hire Part-Time Registered Nurses with 2 years of RN experiences for Case Management within the same Los Angeles and Orange County areas. RN's will be in charge of admitting new patients into the agency, doing head to toe assessments, document changes in patients health/medical condition during the past 30/60 days, document changes in medication/treatment plan within the last 30/60 days, and observe/educate/train the field LVN's that are providing services to our patients.
