1. Unrestricted Social Worker (LCSW). 2. 1+ year clinical or case management experience. 3. 1+ year of inpatient, home health, hospice, public health or assisted living experience. 4. Reliable transportation and valid driver’s license. 5. Computer proficiency; must be able to type and navigate a Cloud-based environment and designated teleconferencing application. 6. Experience with Microsoft Office – Word, Excel, Outlook, etc. 7. General computer savvy - Including basic experience/familiarity with an EMR.
